A New Year, New Milestones: Reflections from Dr. Rahul Jena

As we step into the New Year, there is much to reflect on and celebrate. I am Dr. Rahul Jena, a Consultant Uro-Oncologist at Bagchi Sri Shankara Cancer Centre and Research Institute. Over the past six months, my team and I have achieved significant milestones in the fight against muscle invasive bladder cancer. I’m excited to share these accomplishments, the profound stories behind them, and my steadfast commitment to bladder preservation.

Achievements in Radical Cystectomy

In the second half of 2024, our dedicated team successfully performed 10 radical cystectomies. This remarkable achievement brings us closer to meeting the criteria for designation as a high-volume center, which requires at least 20 such procedures annually. This recognition is crucial, as high-volume centers often lead to better patient outcomes through enhanced surgical expertise and experience.

In addition to performing radical cystectomies, I’ve passionately pursued my primary interest—bladder preservation. Among 14 patients with non-metastatic bladder cancer who needed radical cystectomy, I successfully managed to preserve the bladders of three patients. The remaining nine underwent radical cystectomy, with no major complications or mortality, highlighting the effectiveness of our surgical interventions. Although two patients sought second opinions and were lost to follow-up, their decision reinforces the importance of patient autonomy in healthcare.

A Special Patient Journey

Among the ten radical cystectomy cases, one patient stands out—a 33-year-old male with locally advanced neuroendocrine bladder carcinoma. Faced with daunting health challenges, including severe aortic stenosis, pulmonary hypertension, and uncontrolled diabetes, he arrived in critical condition after a series of unsuccessful treatments. His elevated catecholamine levels were exacerbating his heart failure.

After meticulous medical and surgical optimization, we achieved a salvage radical cystectomy—one of the most intricate surgeries of my career. Despite complications that led to a prolonged hospital stay due to severe idiopathic thrombocytopenic purpura (ITP) and gastrointestinal bleeding, his resolve never wavered. I am happy to report that he was discharged fit and healthy after 43 days.

Heartwarming Patient Stories

Nothing brings more joy than witnessing the resilience and triumph of my patients. Recently, I had the honor of receiving a handmade portrait from one of my patients who had undergone radical cystectomy. Originally from the land of Jagannath, he expressed his gratitude with heartfelt gifts, reminding me of the profound impact we have in our patients’ lives.

Another inspiring story involves a priest who feared his stoma would impede his work in the temple; I’m delighted to share that he is now back in his sacred role. Additionally, a magistrate who also had a stoma returned to court to continue his important work. Each of the nine patients has a unique journey, and their recoveries have only intensified my commitment to bladder preservation efforts.

Gratitude and Future Aspirations

As I reflect on my return to Odisha after years of practicing in London and AIIMS Jodhpur, I am filled with gratitude. I was apprehensive about finding cases upon my return, but the warm support and referrals from fellow doctors in Odisha have greatly uplifted my spirit and practice.

I am fortunate to collaborate with a remarkable team, including medical and radiation oncologists, who share my vision for advancing bladder cancer care. Drawing inspiration from renowned experts like Dr. Ashish Kamat in Boston, I remain dedicated to expanding our capabilities in bladder preservation and radical cystectomy.
